I'm sorry, that was my fault. I didn't understand what you were doing until you showed my your page. Just delete those pages it created and use your original template and the site wizard to create the pages.

 The confusion came on my part came from us discussing the menu structure as if it were a root menu (even though it we never termed that) when in fact what you wanted was a submenu structure. When I downloaded and ran your template locally to help you figure out what was wrong, that was the first thing I noticed - it was referencing a menu item that didn't exist. However, I thought it was a mistake instead of asking you. That was my mistake, not yours.

Anyway, good luck with it.
