I finally got my site up after 13.5 hours here are the step I think did it.

1. Deleted the bin folder

2. Hand Copied contents of the bin folder from the Update package.

3. Set IsEnabled to 0 for all but the DNN AuthenticationType for the table Authentication Contents

4. Reinstalled all modules.

Great summary, Aaron.  And I am glad you were able to get this one patched up.  It goes without saying at this point, but don't forget to back-up everything before the upgrade next time.  ;)

I would have to agree that the number one lesson learned from this little adventure is back up SQL Server.
