I have confirmed the solution for this problem.  When I moved all of my DNN websites into the same IIS Application pool (thus reducing the number of w3wp worker processes to 1) the problem disappeared. 

I've been messing around with my system enough over the past few weeks that my portal admins are quite sensitive to change, so I don't think I'll go 4.0.3 just yet even if it claims to solve this problem.

Thanks again to gaz2sg for suggesting the solution!  Now I'm off to buy a year's supply of Rogain!

Has anyone had a issue where the site just doesnt load with no cacheing?

Yes, I have seen this happen as mentioned in this blog:

I haven't been able to figure out why though.  If you go back into the DB and turn caching back on then recycle the APP you should be good again.

ok but what about the issue of pages and modules sticking around too long?

I have only seen this happen in a web garden (multiple processes in the App Pool) as was mentioned above.  There is a problem with DNN invalidating the cache across processes (no easy task).
