Upgraded from 4.9.0 to 4.9.1 with no obvious issues however I do not see the Dashboard item visible in the Host menu. 

Any ideas on how I can fix this?

go to host settings and hit "Restart Application", this shall trigger the upgrade to re-run.

Nope that too didn't work.

Is there a script I can manually run to insert the dashboard module definition as well the to the host menu. 

I can't seem to find this particular script anywhere in the install scripts.

update logic is part of upgrade.vb, maybe there is a conflict with Dashboard module, which is part of Repository (or another 3rd party component). Is there already a module called Dashboard in your local installation?

I do have the Repository Dashboard module installed may be that was what causing the issue. 

Tried removing the Repository Dashboard module and restarted the application but that didn't help. 

Could you point to where upgrade.vb is because I could not find it anywhere in the distribution in both the install and upgrade package.
