Thanks for the quick reply.

I am looking for a module and will look into these three that you suggested. But do you know if it is possible to remove the submit button and have only a print button on any of these forms?

Binni

If you only have the Print button how do you support to submit the form or are you looking to achieve two tasks from the one button?

Ok, I like the idea of submitting and redirect to a page with my values nicely set up. How would I do that? Can you point me in the right direction?

you can accomplish this with the DNN form and list module (requires dnn 5), or with OpenWebStudio (http://www.openwebstudio.com)

You can also do that with XMOD (under both DNN 4.x or 5.x), though XMOD is not a free option as OWS is.  I create hundreds of forms a year (yes, hundreds) with XMOD so I'm partial to that solution mainly because it's been so reliable to me and it does not generate a new database table for every new form definition (as most other form-generation modules do).

also, alternatively, you might want to look at the form features of PDF Files. Its much easier to create good looking forms that way

For what it's worth:
PDFs are about the best way to go for super-pro looking printed forms and the new versions of Acrobat / PDFs certainly provide some nice end-user data entry, print and online submission options.  Unfortunately if you want to save the data that users enter (i.e., submit completed forms to your site/database) then PDF-based solutions are quite a bit more invloved (and expensive) since you'll need Acrobat Pro for form generation and a special Adobe PDF web server to handle the data from online form submissions.
