Hello.
I am looking for a form for DNN that can allow me to choose to:
1. Submit and send the info via E-mail or
2. Print the form as it has been filled out

The print button should be similar to the submit button.
Do you know of any forms that could fit my needs?

Are you looking for a module to install with a ready-to-use form or the actual code to make your own form?
 
Typically for such tasks I use a "form builder / form manager" module that lets me ealiy create my own forms without writing a custom DNN module.  My personal favorites for making forms are XMOD and Dynamic forms but there are many others.  These modules give you a lot of options for specifying form input fields / questions and arranging things the way you want.  Since YOU create the forms, you can use the modules over and over again for all sorts of form-related tasks (so you're not just buying a single input form for a single task).
 
DNN also has a forms module but it is still relatively new and very limited in scope compared to XMOD or Dynamic forms.  Another interesting (and free) option is the OpenWebStudio (OWS) module for DNN but the learning curve is quite a bit higher for that compared to XMOD or Dynamic forms.  OWS is capable of far, far more than just forms input and is possibly one of the most powerful ways to extend DNN for free without writing your own custom modules.

Thanks for the quick reply.

I am looking for a module and will look into these three that you suggested. But do you know if it is possible to remove the submit button and have only a print button on any of these forms?

with xmod and ows you can possibly do that, but you have to write the "print " logic yourself.

why would you want to print an unsaved form. Isn't it more logical to submit the form, and redirect to a page that lists the entered values in a nice layout, and print that? Printing a form sounds like the internet from the 90's ;) (we DO have a website, but we handle our data only on PAPER)

also, alternatively, you might want to look at the form features of PDF Files. Its much easier to create good looking forms that way
