current version of Feedback module does not support custom fields.

For DNN 5, there is "form And List" module bundled, which allows to create custom form and send by email.

For DNN4 and 5, there is free Helferlein form module at www.helferlein.com which should suit your needs.

Hello,

How can I change the red/white arrows (that denote required fields) into astersks (or something else)?

Thank you,
Alkis

And to add to the last question, how would I edit the html/css to change the look of the form even more? Thanks in advance.

The red and white arrows used to mark the required fields in Feedback 05.00.02 are the standard required.gif image file found in the site's /images folder. It is the same one used to mark required fields in the registration form, etc. I should have specified the image either in module.css or better yet in one of the localizable resx files so that it could be made specific to each portal or skin. At this time, the only ways to change the image are to 1) create a new image file of the same dimensions and name (required.gif) and copy over the one in the /images folder or 2) modify the markup for each field that includes an image tag referencing the hard-coded path " /images/required.gif" in Feedback.ascx. Either of those changes will apply to all portals. The first will also apply to all uses of the required.gif image in other forms as well as Feedback.
