I'm running into the same problem with the same setup (Form Mode). The Feedback form works great and the SMTP test works great.  I entered my Yahoo email address in the Tracking and Notification section (after checking the Notification box above in Mode Settings).  The module indicated "submission successful" but no email notification is received. What am I missing?

Yes, I checked all three triggers just to be sure...
