First of all I would like to appologize if I've started a duplicate thread and/or have placed this thread in the wrong forum.  I'm a newbie when it comes to dotnetnuke and the community.  So far I haven't been able to find an answer to my problem.

Just this week we've started to get this error on our site and portals(so far 3 times) with Exception Details: System.IndexOutOfRangeException: PermanentRedirect.  From what I could find so far, this error has been experienced or associated with version upgrades.  In our case, there have been no upgrades attempted.  We're running version 04.09.04 for a while now.

So far our solution (if you can call it that) has been to restore the database from backup testing to see if the sites comes back up.

If anyone can shed some light as to what the problem can be or help with troubleshooting the problem further it would be greatly appreciated.

I haven't noticed this error on any of our sites (all on DNN 4.9.5). Did you try to restart the app pool? did you configure any redirection in IIS? which OS are you using?

It's running on 2003 server.  We don' have any redirects setup and we have restarted the iis/app pools without any results.  However, looking at the web.config we did notice <add key="AutoUpgrade" value="true" /> & <add key="UseInstallWizard" value="true" />.  I'm making an assumption that the auto upgrade kicks of for some reason.  One of the users mentioned that they saw ...Install/Install.aspx?mode=upgrade before getting the redirection error.  No sure why this would happen.
