Did you recently upgrade the DotNetNuke framework? If so, from what version? It appears that there is a missmatch between the version 5.5.1 code that retrieves the information about a module's registration and the SQL stored procedure which supplies that information. That would lead me to belive that one of the SQL scripts did not run properly during the upgrade. Do you recall seeing any errors during the upgrade (or install) of DotNetNuke?

The site was recently upgraded from 4.9.5 to 5.5.1. I did not see any upgrade errors, but that doesn't mean that there weren't any.

check the packages table in the database, there might be some missing data... Compare it to one of a fresh new local install of DNN 5.5.1
