To answer the original question ....

For complete page output caching, use PageBlaster from Snapsis. It does exactly what you need, and has been around for years. In my experience it provides a significant performance boost to any DNN site.

FYI, it is not a Page Output Caching Provider. It was created long before that concept was added to DNN, and it works as an "HTTP module". There is a free "community" edition and a paid "professional" edition.
