When you say that you can use "my host's File Manager" to successfully create  folders, am I correct that you mean your hosting provider's control panel file manager and not the DotNetNuke site's own  Admin --> File Manager. If so then that is further confirmation that the ASP.Net worker process account (IIS Identity account) has not been granted sufficient permissions (including modify) on the root folder of the installation and/or that those permissions have not correctly propagated to all child folders of the root. In particular, the site's Portals/<#>/ folder and its child folders is lacking proper file system permissions.

Note also that for folders and files that have been created by tools (such as FTP or the hosting control panel's file manager) outside of the DotNetNuke provided interfaces, you will need to perform a recursive file synchronization from the Admin --> File Manager before they can be seen by DotNetNuke.

Ok, that makes sense. 

So I've never heard the of the "ASP.Net worker process account (IIS Identity account)". I assume that I would access that from my host account, correct? Would that be in the File Manager or is it generally a setting off somewhere else? 

(Sorry that I'm probably asking dumb questions. I'm just trying to gather all the information I can to take to my supervisor, because I don't actually have direct access to the host account. If I did, I'd be searching for the solution myself and coming back with more specific questions, if any.)

This is Windows server file system permission that is not set from within DotNetNuke itself but on the web server using either your hosting provider's file or permission manager or if the site is on your own server using the server's Windows file explorer. Since most shared hosting providers do not allow their clients to set the required permissions on the root folder of the installation you will likely need to submit a support ticket with your provider. If the site is hosted on your own company's web server, contact the server administrator to set the permissions for you.

I called my host and they're saying that the issue is not on their end, so I'm not sure what's going on at this point. I found out that I can add folders and files via my host's File Manager. The 'Synchronize Files' in DNN's file manager is working so for now I'm not completely stuck, but I'm still not sure why I can create folders and files in the actual DNN file manager. You said that the permissions has to be set on the root folder of the install. What's the root folder of the install?

The root Folder is the Folder where you put all your dnn files into it.  From this root Folder you have to set physical permissions for the annonymous users with read permissions and for the application pool user with full permissions and inherit it to all Files and subfolders.

Notice:  There can be restrictions with Hosting Managing applications like Plesk, Websitepanel and others.  On Plesk you can not Change the permissions in the rootfolder 'httpdocs' so you have to create a subfolder like 'dnn' - then change the physical path of your Domain into this new created Folder 'httpdocs\dnn' !

I suggest to use DNN 6 or 7 last Version by security issue. DNN 5 is a very old Version !
