I have been using HostForLIFE NopCommerce Hosting foraround five years. I am happy with their price and their quality. I can not say they are cheap or the best. But, I am happy with their support. I take a different approach, if you want to pay more but get DNN specific support go with powerdnn.com or managed.com.  In my case I have my own Window's 2012 server (Hyper-V) with 4GB and 4  Core dedicated, 2 IPs, unmetered bandwidth for less than 70$ a months. being a Microsoft BizSpark member I have a product license for SQL Web edition 2012. (it is worth signing up with BizSpark or WebSpark).

If you are US based go with a US based server (in my case cari.net) and if you are Europe or Middle East based go with host in Europe, (I had excellent experience with redstation.com). Or alternatively Microsoft Azure is a very good choice.

First decide on your approach based on in-house experience, time, resources planned scalability, the rest will follows.

I can tell from experience DO NOT use Windows Azure to host DNN... we are actually in the process of reverting our production instance off of the DNN Cloud Services (which was drive by Windows Azure) because the performance is HORRIBLE.  We have struggled with it for almost 2 years with DNN making tweaks here and there to try to help it.. nothing we did worked... plus, on this platform... if  we made any configuration changes or installed a new module... the site would have to restart... and it would take 30-45 minutes for the site to become responsive again.  Needless to say avoid the DNN Cloud offering all together.. 

We are reverting install back over to a RackSpace server... which with our testing seems to be performing really well...
