I am also getting this issue after upgrade to 7.4.2 and will try out the suggestion above.

I am wondering if this might be related to HTTPS. We only recently switched this on (in version 7.4.0) and I am wondering if there is something related to this .

Not related to that, if user has open session left in browser this issue occurs always.

I have to point out that the advise "Tell your users to..." will usually be impossible.

If someone is already a user then they are unlikely to register another account.

If they are not a user then I don't know whom they are yet. Therefore I cannot contact them.

I was actually today trying to find a root cause and almost did it as well :)
