So, one of our editor/admins was going through the file manager to clean up unused files and then came across the User file in the root folder. Inside was a bunch of numbered folders with even more numbered folders inside. However, everything was empty with nothing of value inside. My question is, what are these folders being created for and is it safe to delete them?

I would not delete them. These folders normally hold profile pictures and other uploads from the user. They are essential for the core functionality - even if I cannot explain the meaning of a structure like /001/01/1 ;-)

Even if that functionality is not enabled or used on our site?

you may safely remove all empty user folders - DNN will recreate any of ot, whenever this user logs in.
