Does anyone have any idead why I see the install/install.aspx page everyone once in a while? Here is the text:

Upgrading DotNetNuke
Current Assembly Version: 04.00.02
Current Database Version: 04.00.01

Upgrade Status Report
00:00:00.078 - Performing General Upgrades

Upgrade Complete

I know the db and code are different versions. Could this be causing it? I think the db structure is exactly the same though...with the exception of the version number....

Whenever the Assembly and Database are at different versions, the AutoUpgrade code will run to "upgrade" the Database.

In this case from 4.0.1 to 4.0.2.
