Very carefully.

I'd start by doing a LOT of testing.

Pull the site down to a machine to test on. Then try the upgrade, be sure to properly configure your web.config file. You can also checkout my blog here on dnn.com for a post or two about upgrades.

Sory the mistake, is to upgrade to 4.5.1!!

Should I go direct to 4.5.1 from 3.0.13 or sould I upgrade to same version before 4.5.1?

My Modules will keep running after upgrade? What sould I take care about modules?

That kind of depends on what modules you're using. Most modules, if properly developed, should still work. Though if you have any registration/user profile type modules they will most likely not work as there were major changes to the user profiles over the versions.

Your best bet would be to test the upgrade to 4.5.1, if it fails, start over and test another version.

I would try 3.0.13 -> 4.5.1

Then I would try 3.0.13 to 3.2.2 to 4.5.1
