I have the exact same problem: I've upgraded from 4.9 to 5.1 and then to 5.4.2, and the extensions menu is missing from the Host menu, and grayed out from Control Panel. The Module Definitions menu is now empty. Rolling back to to the 4.9 install is not very appealing, I'll have to upgrade my database from sql 2000 to sql 2008 again. Is there a way to run an sql script to fix this particular issue. It seems that a lot of people are having difficulties making the transition from version 4 to 5, maybe some hotfixes to each of the more common problems people are experiencing would be helpful?
