It appears that the following file which would have been used in the 4.4 version , located in the /bin folder of the site installation, did not get removed during the cleanup phase of installing the Html module:

DotNetNuke.Modules.Html.SqlDataProvider.dll

If it exists in the site's /bin folder, delete it and see if that corrects the problem.

Did you upgrade directly from 4.9.5 to 6.1.2?  If so, be sure to check all functions of the site, particularly administrative functions, to see if they work properly. That is a rather large jump in versions. My recommended path is from 4.9.5 to 5.4.4. to 5.6.3 to 6.1.2 with some others recommending even smaller upgrade steps.
