This was one of the major problems I had in my disastrous upgrade to 4.3.3.... (sorry - no solution as yet)

I experienced the same problem and the following steps resolved it. 

1) Log in as host

2) Click on the File Manager icon

3) In the upper-right hand corner select "Recursive" and click "Synchronize Folder"

If you are still having problems. I believe that the version of the Document module is important. It does not install if you use the DNN 4.3 upgrade package. I experienced access problems as well. The new version of the documents module seemed to solve the problem. It is available on the DNN Projects page.

I had the same problem. Yes you will need to log in to the file manager and sync your files, be sure "recursive" is checked. The larger problem for most of my users was the duplicate folder. (//)

I ended up going into the db, deleteing the files associated with those folders from the db, (Files Table) then deleting the folders from the db (Folders).

The real probelm here is that I upgraded a pair of small installations first. I have several installs with 50+ sites on them that need to be upgraded, and I have no desire to wrestle with this problem there. An equally important question for me is how to eliminate this problem on the front side when upgrading.

I believe you could truncate the files table and then the folders table, then recursive sync and it might work, didnt have the stones to try it.

Hmm - thanks for the advice, but this did not work for me. Nothing happened after I synchronised and I have no "//" after any of my folders. I am using the latest Documents module...

Just so you guys are clear - the "//" and permissions errors when downloading files are not a "breaking change", they are a manifestation of bug.   Scott W has posted some blogs that may be of help:

The change to the documents module around security was to support the new "secure storage" features, and to not display documents that the end user doesn't have view rights to.  This change has nothing to do with the "//" or "access denied" issues.
