For some reason When I edit a child portal alias

e.g. mysite/portalA to mysite2/portalA  the site will load

if  I do this 

e.g. mysite/portalA to mysite2/portal_A  the site will not load - simply get a page not found

Anyone run into this?

I check the portalAlias table and the values look ok there

for each child portal a subfolders with redirecting default.aspx is created. this is only done on creation not on updates. you therefore need to adopt folder name and content of the default.aspx your self (use a text editor to edit .aspx files).
