There are several third party modules for interactive forms creation at snowcovered.com. Note: DO NOT Purchase the one by R. Cox as it doesnt function proper and frankly... it sux.

I am actually working on a forms module but its in early stages. We had to already go with a "forms" item for a few DNN sites and decided to Dreamweaver along with a send mail (Jmail) for the easy forms.

Remember "Folks" with DNN you do have the magical iFrame module. That means you can use LOTS and LOTS of things out there and put them in iFrames. As another example... One site we are working on needs streaming flash video. We decided upon Coffee Cups Video Player, it creates a HTML web page, that then goes in an iFrame. Works wonderful, requires no Flash Server. Another advantage of this (with forms for example) is your not eating up Database space, your not pushing on the server CPU for DB Access/ASP processing... Its just plain ole' vanilla HTML and this IIs pumps it right out. Of course, there is the DNN iFrame overhead involved but.... tell ya what, we have really had great results with it. Our host has PHP mounted as well as ASP.NET so we can happily run PHP stuff in an iFrame. We did this with "Chat" on a site as well as we could not find a good chat component for DNN, I guess they are making one.
