Can anyone please suggest how I may create an application  form to prompt my portal users
o fill in a registration type form, for applying to a course?

It seems backward to create a printable page with brackets and text. I guess the ideal would
be to store the user data in the database and email the results to the course provider. At the
very least I'd like a presentable application form, from which the data, once submited, can be
setup to be automatically emailed to a designated person(s).

I was after something a lot simpler to implement. I'd like to start with a simple form containing a couple of text boxes
asking for name and phone number, email (optional), and a dropdown, or list with checkboxes to select the course
desired.

That isn't what I am trying to do. Once a user is logged into my portal, I wish to put a request
for further information page in, selectable from the menu. This page is where I'd like the user
to have the option to insert a email different to their login one (they may even be logged in as
guest)), and be able to select an item from alist of items, that they wish further information to
be emailed to them about.
