All sites have been upgrades to date, some from v3.x. I have not tested this on a fresh installation. ALL 4.5.1 sites have this, even fresh installs. Upgrades from 4.5.1 to 4.5.2 are showing this. Right now basically even configuration we have has this problem.

The upgrades have shown sqlDataProvider errors, those from 4.3.7 to 4.5.1 (see here).

This is pretty wicked even though the perms are not adjusted. Our users are excited about the granular control but now we are buried because of the "All Users"  issue.

This has been verified to be an issue in DNN 4.5.0/4.5.1, that shall be fixed in DNN 4.5.2. It looks like either the migration to 4.5.2 has an issue or the display in the permission grid. We will have a look into this and let you know.
