I'm looking to set up an anonymous survey and need some way to collect comments from users.  To ensure one vote per voter I'm having everyone log in before taking the full survey (consists of multiple questions/survey modules).  The only problem is that the feedback module displays the user's email and display name - otherwise it would be perfect for this.

Is there a way to remove the email and name on the feedback module?  I'm not referring to the moderated part of it; just to the part where you type your message.  I'd like to not have the email and name echoed on screen to the user in the area where they're typing the message.  Is there a quick way to do this?  By the way, I'm running DNN 4.5.3 & the latest feedback module version.
