I remember this from earlier versions of DNN.

Then it had something to do with caching on the server.
If you clear the cache and restarted the application they started rotating again.

The thing with users online might be because they only get updated every x minutes.
You can check this in the scheduler.

When you clear the cache and restart, it works for awhile and then it reverts back to a locked position.  It locks when a certain set of banners all all showing at the same time. (My site has multiple banner locations).

We are having that exact same problem with about 8 sites we converted from DNN 3 to DNN 4.7. We can get it to work temp. by clearing cache, etc. But it eventually returns to a locked state with one or two banners.

By chance are you using PageBlaster (snapsis.com)? I had the same problem. If so you must exlude any pages using the banner module.
