How can I create a Get a Quote or Contact Us Form in DotNetNuke?

I used Matt's Form Mail script in an HTML IFRAME MODULE to create the Get a Quote Form like the one below and to the left but would rather create a form using DotNetNuke like the one below and to the right.

Get a Quote (Matt's Form Mail Script)

Business
City
State
Zipcode
E-mail address
Phone number
Any additional comments you have
please include & let us know how
you would like to be contacted

*Note: All fields marked with a red arrow are required.

In order to use forms in DotNetNuke you must use a form creation module.

My company uses XMod (http://www.dnndev.com) to create our forms. They have a lot of very nice options and the price was right.

There is also ListX (http://www.bi4ce.com) that is extremely flexible, but I've heard the learning curve can be a bit steep.

I would suggest digging around on snowcovered.com to see if you can't find a form module that will fit your needs.

Not aware of any free user registration modules.  What particular features do you need that are ot available in the core user registration module?
