Is there a way to set up a form for users that would create a child portal based on a specific template when they hit submit? Is there an existing module that allows this or a configuration setting for it?  I would like to allow some of my users the ability to do this without giving them host privilages. Has anyone done anything like this?

Yes, check out Onyaktech's Portal sign-up. There's a demo site here http://www.freednnsite.com/

The unique feature of that module is that the user doesn't have be granted admin rights.

Rob, if you're using this module, can you please tell me how the user who is not granted the admin rights can edit his portal? I tried this function once but couldn't get it to work as expected.
