I'm noticing some customers are having logins like USER48bebec6fb9b4bb5. How can that happen? How can user create such a user name on the registration page?

AFAIK thats a valid username as it's 20 characters long, I would suspect that it's not a genuine user, perhaps someone trying to access some resources. Some sites choose to enforce valid username's by marking registration as verified or by setting the requiresUniqueEmail attribute to true in the web.config.
