I need a standard type of feedback form for several fields,such as - name, company, address, comments, where did you hear about us?  Can't get the standard feedback form to do this, any suggestions?

I'll recommend Interactivewebs' Complete Feedback Designer for the purpose. It fits nicely between a basic contact form and a complex form designer, by simply letting you add all the form fields you like. The big form designers are fine if you're happy coding, but overkill otherwise.

Helferien forms, linked above is great for quick and simple forms that send emails.

Another forms tool I like is H20, for more sophisticated forms, that will also write to a database table.   Onyaktech
