Hi All,

We want to setup a contact form on our site.  Looks like the Feedback module would work, except that we want to add some additional fields.  For instance, instead of simply having a box that says "Comments", we might put a separate field for address, city, state, zip, phone, and other specific question (what services are you interested in?  What is your timeframe for purchase?  Etc.)

By asking specific questions like this we'll get much better information.  Also, it would be nice to be able to make some fields required (but not necessarily all).

Is this possible with the Feedback module, or another Core module?  If not, how about a free module?  If not, does anyone recommend a commercial module?
