While there seem to be many online survey/questionnaire creators (like http://www.surveygizmo.com/ for example) which allow users to easily create an online survey in a few steps, I hardly find any similar module for DNN. formbuilder solutions does not fit our needs as they do not provide conditional logic (e.g. someone answers a question with 'no' he will directly go to question no. 8) and our users would have to build a form (put textbox here, put dropdown there, etc.) but not a questionnaire (e.g. new text question, new pick-one question, new priorize-question, etc.)

Can anyone recommend a survey/questionnaire module which enables our publishers to easily set up typical surveys/questionnaires?

Have you checked out our Enterprise forms module?  It has the exact capability of conditional logic evaluation you're looking for.  If you have specific questions, feel free to post our community forums.

Hi Ethuongmai,

thank you for your answer. I had a look at your forms module before, but as mentionend above, we are looking for a Suvery/Questionnaire builder, not for a form builder. We need to enable our eemployees who usually are business people without an extensive IT background to easily and fast setup new surveys. They don't know anything about "rows", "columns", "alignments", "CSS", "HTML/Text", "RadioButtons" etc., but that they want to have five yes/no questions, two free text questions, a field to type in the age, six questions with a rating scale from 1 (very good) to 6 (very bad) etc. If we would implement a form builder then again it would be our IT staff who had to setup a questionnaire for our business units which again would cause an additional workload. Tools like the above mentioned SurveyGizmo provide an easy to use GUI where users without any extensive IT knowledge can create surveys by picking from common question type like the above mentioned.

I agree, there's a gaping hole in the DNN module scene for a dedicated survey module.

There have been two candidates in the past, but neither is suitable. Rhema Concept's Super Survey requires that the survey creator is granted page edit rights, the other one, by DNNToolset, looks good, but may as well not even exist, given the unresponsiveness of the developer.
