Hi Sebastian, It does seem to point to permissions. When I do an install I use Plesk panel to create a database and user. this has always worked in the past without issue, then one day it just stopped working correctly. I will investigate the DBO permissions you mention to see if I can spot anything.

OK I just did a test install which worked perfectly. After creating the database and user in plesk I went into Management Studio and granted db_owner to the user. I then ran the install using the custom DB method (which has 'run as DB Owner' checked by default) and the install ran without errors. Not sure which of those steps was the fix.
