Sometimes a active virus scanner process can cause this issue on heavy loads. Try to exclude resource folders from real time virus scans. It helped on my portals on similar issues.

I'm not sure if its related but I had problems at first with cache files. I had scheduled purges of the cache files but they were not happening - turns out it was a permission problem.

I gave the domain user full control and also gave each machine account running IIS full control to the share. That seemed to fix the problem.

Bradley, I'm using Vice Versa and wonder if you still are.

I have two webheads, should I be syncing all files, including cache files to both machines?
