In an effort to provide users a way to let me know of bad content, I need a skin object and/or module that will put a link in the container of all the modules on my site that links to basically a survey module allowing the user to submit a complaint/request/comment about a specific module. 

In short, I want a "report abuse" button on all my modules that allow a user to tell me that something is wrong, and it will automatically tell me the tab and module that they are reporting on.   I have seen a site that has an example of what Im looking for, but I can't seem to find it again. 

Anyone know of any skin object/module combination that does this, or another way to achieve the same result?
