I have tried using the code in the html editor and in the view control source code and it doesn't work in either. Also, I don't know how to check the mime type of swf is enabled for the web server - I am testing it on the local file server via visual web developer so do I need to make any mime type adjustments here?

I believe the answer is Yes. The local server needs to have all the required mime types set and swf and flv are not always default settings.
I had the same situation, code that should be working and a blank box of the size of the file.

Actually I have never resolved this locally however when I put my page code onto my web server and enabled the right mime types it all started working.
I should qualify this by saying I no longer use local host for testing as it is too different to my deployment scenario and I use VisWebDev only occassionaly.
Can I suggest that this may not be a DNN issue and that it may be useful to have a poke around on the Adobe Flash forums.
I also found this website useful: http://www.jeroenwijering.com/?item=Embedding_Flash
