Is there any core module that can do the following:

Scenario - when new employees start, they need to request business cards.  I would like a form on our intranet that the employee completes by filling in the appropriate fields (name, title, email, phone #, etc.) and have that completed form emailed to the person in charge of ordering business cards.  I have another scenario that could use a similar form setup and could see this being usefull for other things.

The Feedback module comes close, but you can't have particular fields that need to be filled out.  Or can you?
