Is their a simple custom form module out their. I have tried Xmod and H2O, but both are very complicated to set up. I surprised their is not something simple. I just need a mod that can easily add textfields or drop-downs to a feedback form. Being able to store results in the db would be a plus. I'm really surprised I can find one.

I use ods.form http://www.nukenow.com/Mymodules/ODSForm/tabid/347/Default.aspx

Very simple to setup - also saves feedback to DB or sends via email or both.

Works fine in DNN 3 and 4

Form Master is certainly worth a look.

I have 3 copies of Formmaster now. Very good, nicely abstracted and its quite easy to make forms actually look good. The file upload is always useful too. A solid bit of work.
