I am hopping someone can help me out here.

 

We are still using a version of DotNetNuke 3x, and I am having a little problem installing a module that I had created a year ago, and I know was working when I created it.

 

When I try to upload the module to the DotNetNuke portal I ge the following output

Does anybody know what is causing this error, and how I can fix it

Ok looks to me like you are missing files in your zip.  The installer is trying to create the sqldataprovider but your zip does not appear to have one. 

Also if this is a DNN 3 module why do you have the .vb file included??

I would open this project up in VS and check everything is correct.  Looks like your module intaller is not complete.
