Another form builder that also has great grid view functionality is IndooGrid (available at Snowcovered). The 'form' part would be their GridEdit and any reports you'd like would be their 'GridView' part of the module. The other easy answers have already been posted (XMOD, XMOD PRO, OpenWebStudio, DotNetMushroom).

All of their modules cost $99 i think.

They have a form product called H2o you can build as many forms as you like

You should also take a look at the Form and List module (a core module). It works well for some forms but there are some limitations when it comes to styling the form.

Hi,

If you don't need grid view but only custom forms, our module SimpleSignin could interest you.
You could make simple contact form and allow users to register on your site.

It all depends on what you are after, if you just want a form builder, then there are some good modules that are good at building forms. Whilst our DotNetMushroom RAD is good for building forms, it is also good for building more complex applications. Obviously I will not rant about its flexibility and efficiency and so forth because this is a place for the community to learn. Moreover, I will not criticize other products because it is highly unethical to do so. What I honestly recommend is to download a trial version of such modules and try them out yourself.
