It would be great if you can post your own issue on a new thread.  To look about a solution for your issues, first check the Forums, wiki, Blogs if any Solutions are similar to yours and could fixed your issues.

If they not fixed your issues please open a new thread for your issue.  Thanks.

Looks as you have double entries into your database and missing contrains.  This could happend on a issued Installation before this upgrade Task.  So you should restore the old files and database from your backup, check your DNN Environment about issues and fix them first.  Then restrat the upgrade process from cratch. 

Your issues Looks as they are located on the HTML Module and database artifacts.
