Looking for a module (preferrably free) that allows for the creation of a custom feedback/email form that allows you to define custom fields so that that user can fill out the form and have it emailed to a set address.

I know i've seen/used one before for DNN2 but can't seem to track it down for DNN3-4.

Anyone know of one?

I don't know of a free one, but Xmod will let you do this and more. Its worth the few dollars it costs.

Thanks.  I bought Xmod but never used it as it seem to be rather complex to do much in, powerful yes but overly confusing to use.  Seems like a lot of extra overhead and overkill for this.  I just need something more simplified to create a custom feedback form to take customer quote requests and related.    I was considering the "Dynamic Forms" module.  Want something fairly simple and focused on the task at hand, not a do it all module with a steep learning curve, otherwise it is jsut as easy to build a custom module to handle it.

Xmod looks a little intinidating at first, but once you build a couple of forms its fairly simple. The forums over at dnndev.com have a great bunch of people around and they can answer any questions you may have. Since you already have it, take a look on the site at the repository, I'm pretty sure there are forms already on there that will do exactly what you need you just may need to change the text and the email address. If you will shoot me a mail to bhopkinsATchattcollege.com I'll send you the form from http://www.chattcollege.com/Default.aspx?tabid=35 its xmod 3.x and it changes the email its sent to based on the selection in the drop down. Has worked great for me.

I should mention I have no affilation with Kelly Ford at all, I just think he hit a home run with this module and there is nothing like it. I tried a couple of the other form builders out there and none of them work as good or can they be set up as easy. I won't mention any names as the other products are decent just not as good in my opinion.

Thanks Bruce.  Does Xmod handle required fields and such?  I assume it would jsut all the email forms sampels I've seen never seem to use it, you can hit submit on an empmty form and it happily sends it.
