I upgraded 3 portals to 4.5.3 from 4.5.0 and two from 4.4.1. All is fine except when I try to update or add an image in the FCKEditor 4.5.0. When I click the insert image button the initial dialog box opens fine. When I click browse server the dialog window opens and the background is loaded with thte gallery name but the content (folders and images) is an empty white space. I tried reinstalling the module with no change.

The provider is not a module and cannot be installed as a module. What version is the one your tried?

I have exactly the same problem.  I have two portals, one on 4.05.01 and the other on 4.5.3.   The above scenario works in the portal with the older version of DNN but in the portal with the latest version, all I get is a blank screen when it should be showing me all of the available images.

Did you manage to fix this problem?

I've just upgraded to 4.5.5 and the problem still exists.
