There might be another module for you that is free and open source called "Dnn Quick Form" found here:

If you have a basic knowledge of asp.net forms ( which is not so hard ) you can create layouts of the form itself.
A nice little form for contact forms.

The main open source free module is called "Form and List" which can be found here:

https://dnnfnl.codeplex.com/

This module can be used for more than just creating a contact form, also worth a look at.
