The issue must be more than just the DotNetNuke.dll file being of the wrong version. Let's see what the actual error is:

1) Make a backup copy of the site's web.config file.
2) Open the site's web.config file for editing (download and then open if site is remotely hosted).
3) Seach through web.config for the text:

<customErrors mode=

4) Replace "RemoteOnly" or "On" with "Off". Note that this is case sensitive - must be capital "O".
5) Save the modified web.config (and upload back to the site's root folder if remotely hosted).
6) Revisit the site. The actual error message should be shown.
7) Post all of the error message here for me to look at.
8) Until we get this resolved, you may wish to replace the 4.09.05 DotNetNuke.dll file with the original 4.09.01 DotNetNuke.dll file that was in the site's  /bin folder before.

Here's what I got.

It looks now like there is a mis-match of versions between the DotNetNuke.dll and DotNetNuke.SqlDataProvider.dll files. The DotNetNuke.SqlDataProvider.dll file from the 04.09.05 Upgrade package has a file/product version of 3.3.0.19084. Can you check to see what file version the site's /bin/DotNetNuke.SqlDataProvider.dll file is showing in it's properties? You may want to do some other comparisons of dll assembly files currently in the site's /bin folder with those contained in the 04.09.05 upgrade package.

Is there any chance that at some point a file system restore from an older backup (taken when site was on 4.09.01) might have caused this issue?

I'm not that experienced to attempt to make any kind of change like that. My host provider at one point had a server crash and needed to restore this site. It was shortly after that restore I started experiencing problems. How do I determine what version is the DotNetNuke.SqlDataProvider.dll? I'm using Dreamweaver CS4 and it has no option for viewing properties.

please unzip all files from DotNetNuke 4.9.5 upgrade package and copy hierarchically into your DNN installation directory, replacing all existing system files.
