Is it possible to develop a DNN 4.x module with the DNN 5.x Visual Studio Starter Kit? If not, is there some way to run both versions of the starter kit on the same instance of Visual Studio?

The starter kit is only a template, so yes, you can, you just have to be sure that the module, when compiled is built to the specific "Minimum Version" that you would like to support.

For example if you want to develop a module for 4.6.0 and later, it must be compiled against the 4.6.0 version of the DotNetNuke.dll file.
