Where to get the source code for DotNetNuke.HtmlEditor.TelerikEditorProvider.dll?

AFAIR it was the editor used from 5.4.0-6.1.2 (http://www.dnnsoftware.com/wiki/Page/HTML_Editor_Providers/NoRedirect/1) so any source build of those versions (see https://dotnetnuke.codeplex.com/releases/view/112531)

I'm using DNN 05.06.06.

you should Switch to RadEditor Provider or CKEditor (http://dnnckeditor.codeplex.com),
TelerikEditorProvider has been sunsetted.
source code is contained in Source packages on Codeplex.
