A portal I support was suddenly missing a module on the homepage today.  I went through the database to see what happened and I can't find the module in the Modules table.  The module id isn't there.  I do find the module id in the TabModuleSettings and the Permissions table.  How would a module be removed completely from the modules table?

I went through the log and no mention of the module being deleted as well.

this really sounds strange, especially since there should be FK Constraints from Modules to TabModules and ModulePermissions.
are those FKs not in place?

I agree, it seems very strange.  Is there a log in SQL that might shed some light on when the row might have been deleted?

you should be able to enable logging of this Event in Eventlog > Log Types, AFAIK it is disabled by Default.
