Sanjay a follow up on the last post - all I'm trying to do is add a simple contact response form to my contact page - one like this:

Is there another core module that should be used?

David - the feedback form is basically to submit feedback to the the site administrator or some other email address that has been setup. Looking at the link you provided, it looks like they are using the feedback module in addition with a text/html module.
