I upgraded to version Survey 4.5.0 and the settings page does not come up.  (that is, a blank page is shown instead).

The log viewer shows:

What's even more interesting is that now the Event Viewer fails to load roughly 66% of the time with the following exception when it finally does load:

If I were going to change the "settings" in the database by hand, what table would I look in for them?

If you upgrade to the latest DNN this should go away.
